Check each voice coil at the speaker first. If they check fine then it is most likely the amp. As Adrian said above probably a driver board or a bad trace on the board.

Do you have another amp to try on the subs. Sounds like the amp itself as it's intermittent. Maybe contact Soundstream Technical.

Because it has alot of parts in it... It's a ~2000-watt total RMS power Class A/B amp -- naturally it's enormous. Ok maybe Im trying to compare apples to grapes here but The leviathan is 150rms x 6 channels. 200.4 is 50watts more, but 2 channels less. I dont understand the HUGE size difference. The SAX-200.4 is also 1 ohm stable. Bigger heavier duty internal parts make for a large footprint.
Meaning 3-4 weeks you start shipping them out? Or 3-4 weeks until you can start making them. Products should be in stock and will be shipping out.
The IA 20.1 would be a great amplifier for a 21" DP. If you wanted more power then get a 2nd 20.1 and strap the pair on a 21" Warden for insanely loud daily driving. Just adress the electrical!

Sub firing up and port either up or to the rear. If firing the port rear make sure you have enough room for the port against the back!

I would do rear firing. I did this type of enclosure in a friends Nissan Maxima and it sounds excellent. Find the irritating rattles and add some SDS products.
SA-12 in a 1.5ft^3 ported enclosure tuned to 35hz with 1 a 4" aero at 16.5" long side firing into the trunk corner. Run the Orion-1200D at 2 ohm with upgraded Big 3 and you should be golden. As far as a LOC may want to search on that one.
